This form allows parents or legal guardians to authorize their child over the age of 12 to attend Echo Ridge Recreation Club without direct supervision. By submitting this form, you are giving permission for your child to access club facilities independently and acknowledging your responsibility for their behavior, safety, and adherence to all club rules while on the premises.

I consent for my dependent minor to attend Echo Ridge Recreation Club (ERRC) facilities while unaccompanied by an adult. I understand that this is a privilege that may be revoked if my child fails to obey the ERRC Bylaws and ERRC Rules and Regulations.

Our Lifeguards are responsible for maintaining the safety possible environment, but they cannot and are not expected to act as babysitters or child care providers. I understand that neither ERRC nor Dynamo Swim are responsible for the supervision or care of my dependent minor before, during or after my dependent minor is in attendance at the ERRC facilities.

I have read and agree to the ERCC Rules and Regulations. I understand that my dependent minor may be asked to leave the ERRC facilities if they fail to obey ERRC Rules and Regulations and/or ERRC Bylaws.

I have provided and agree to maintain current contact information below and on my online membership profile at echoridge.memberspash.com. I have provided another emergency contact listed in this form and will be available at all times that my child is in attendance at the ERCC facilities and not accompanied by an adult.

Excerpt from ERRC Rules and Regulations:
With parental consent and subject to the ability to obey ERRC’s rules and regulations, minors who are age twelve (12) and over may be in attendance at ERRC’s facilities without an accompanying adult. ERRC and the pool management company bear no responsibility for the supervision of unaccompanied minors before, during, or after the time they are in attendance at ERRC. Parents who consent to their children’s attendance at ERRC without an accompanying adult also consent that their children may leave the pool, either by their own choice or if asked to leave for other reasons, without an accompanying adult.

An emergency phone number must be on file for all unaccompanied minors. A parent or guardian must be available by phone at all times in case of emergency or any other matter deemed necessary for their notification.

I certify that I am the parent or legal guardian and authorize my minor to be on ERRC premises unaccompanied. I accept responsibility for their actions and acknowledge the club’s policies.
